Instrument Rental Form

• Please complete the Rental Form for School Band and Orchestra below.
• There are several instrument choices available to students. The monthly rental fees are below.
• Rental payments are due on the first of each month. Automated account processing is available.
• We’ll contact you when the instrument is ready.
• Please bring a Valid Drivers License.
• A minimum rental of two months is required, after which the instrument may be returned in good condition, with no further obligation.
